Please bring the following documentation to be submitted with your 2025/2026 Pre-K application:
Child's birth certificate
Child's SSN
Parent's ID
Proof of Georgia Residency (utility bill, car registration, lease)
Immunization (Georgia Form 3231)
Ear,Vision,Dental,Nutrition(Georgia Form 3300)
